Position Overview
The Independent Living Coordinator provides the five core services: Advocacy, Skills training, Peer support, Information and Referral and Transitions to individuals with disabilities who are either living or transitioning towards living in the community.
Responsibilities
- Assessment of an individual’s ability and service needs to live independently.
- Provides information and referral services.
- Provides Independent Living skills training.
- Supports consumers in advocating for themselves.
- Coordination of services with state and community-based agencies.
- Supports goal setting and the development and review an Independent Living Service Plan with consumers.
- Participation in community outreach and advocacy representation activities of the center.
- Documents services and activities in database in a timely manner.
- May be assigned to work on specialized projects or contracts including but not limited to: supporting individuals with employment goals, coordinating long-term supports and services, supporting individuals with mental health and substance use disabilities, working on systems advocacy and/or community access surveying.
Qualifications
- Bachelor’s degree in a human service field, or equivalent work experience.
- Demonstrated ability to develop and implement service delivery systems, policies, and procedures.
- Excellent verbal, written, and interpersonal skills, including the ability to interact with diverse populations.
- Personal knowledge of disability issues preferred.
- Working knowledge of Windows and Microsoft Office applications.
- Fluency in Spanish, both verbal and written, preferred
- Ability to make home visits where physical access may be problematic.
- Must have own reliable transportation and current MA driver’s license.
Hours
- Full Time
- Day shift: Monday – Friday
- In-Person
Salary
$42,000 – $44,000 annually, depending upon experience
