Janna Schrandt – Community Transition Coordinator

Janna joined MWCIL as the Community Transition Coordinator with an additional role as a Care Coordinator for CCA (Commonwealth Care Alliance) on April 5, 2023. 

She has been involved in Social worker for over 20 years and holds a master’s degree and licensure (LCSW) in the Commonwealth of Massachusetts. She worked at WestMass Elder Services and Springwell as a Case Manager/SCO, in their PCA program and as an Assessment Specialist. Janna also worked in various other fields such as hospice and mental health as a clinician/substance abuse/dual diagnosis counselor.

When Janna worked at MWCIL in the past, she helped initiate the Options Counselor program.

Her current role is Community Transition Coordinator where she helps individuals with disabilities who been residing at an institution for a while to transition back into the community. This transition is achieved by finding accessible and affordable housing, and performing a needs assessment to determine what services are needed for a safe, smooth, and successful transition back into the community.

As a Care Coordinator for CCA, Janna works with members and CCA Care partners to help the members live safely and independently at home, with the necessary services and support in place.
